We are seeking a Family Practice or Internal Medicine Doctor or Nurse Practitioner to complete the health history and perform physical evaluations of psychiatric patients admitted to Rainier Springs.   You will be a member of a multi-disciplinary team, in which you will communicate any medical concerns at time of admission and throughout the patients’ treatment.

Position Details:

  • Oversees the quality of care delivered to his/her patients.
  • Assesses patient(s) and complete health history and physical evaluations in a timely manner.  Addresses medical concerns at time of admission and throughout the patients’ treatment.
  • Communicates medical decisions to staff to ensure appropriate care.
  • Maintains patient confidentiality and complies with all HIPAA standards.
  • Completes all documentation requirements legibly and within timeframes established by Medical Staff Bylaws and Rules and Regulations.
  • Participates in the multidisciplinary treatment team and participates collaboratively in scheduled staffing regarding the treatment of his/her patients.
  • Demonstrates appropriate medication administration and ensures that documentation regarding medications complies with facility and regulatory standards.

Requirements:

  • Graduate of an accredited program required; board certification preferred.
  • Current, unencumbered MD / APRN license in the State of Washington
  • CPR certification and Crisis Prevention Training (CPI) preferred and obtained within 30 days of hire. Successful credentialing with the facility.

Family Medicine Physician: Silverton Oregon

Keizer Clinic – Position # 17-2976 this position is located about 15 minutes outside of Silverton Oregon

Clinic is a Primary Care Clinic with Internal Medicine and Family Practice providers.  This Practice would provide primary medical care to all ages from infant to end of life. Clinic call is required but hospital has a full time Hospitalist program … ED and IP call is not required. Clinic is staffed with skilled MA’s, RN Case Manager and a Behavioral Health Consultant.

Keizer is a rural farming community of approximately 37,000 residents located in the mid-Willamette Valley, 15 miles from Silverton and Woodburn and 30 miles from Portland.

Woodburn Health Clinic Position #18-0014

The practitioner would join a clinic of 9 providers, with a full panel already established.  This is a multicultural community with vibrant Hispanic and Russian populations.  Point of care testing is done and the health center is staffed pharmacy, mental health, rehab and a wide array of specialists.  Clinic call is required but ED and IP call is not required.

Providers for both positions would be on medical staff at Legacy Silverton Medical Center.
