Deadline to Apply for Making Care Primary

Washington is one of eight states selected to participate in Making Care Primary, a new primary care model devised by the Centers for Medicare & Medicaid Services. The 10.5-year model will seek to improve care management and care coordination, equip primary care clinicians with tools to form partnerships with health care specialists, and leverage community-based connections to address patients’ health needs as well as their health-related social needs such as housing and nutrition.

Though the model lasts more than a decade, there is only one chance to apply. In response to stakeholder feedback, CMS extended the application deadline to Thursday, December 14 at 11:59 p.m. EST.

The MCP application is non-binding and should take no more than 15 minutes to complete. Eligibility requirements and additional model details can be found in the Request for Applications. To support interested applicants in their decision-making, CMS published an Example Revenue Factsheet which compares potential revenue under Medicare fee-for-service to potential revenue under MCP. Please visit the model webpage for additional resources.
