Deadline to Apply for Making Care Primary
Washington is one of eight states selected to participate in Making Care Primary, a new primary care model devised by the Centers for Medicare & Medicaid Services. The 10.5-year model will seek to improve care management and care coordination, equip primary care clinicians with tools to form partnerships with health care specialists, and leverage community-based connections to address patients’ health needs as well as their health-related social needs such as housing and nutrition.
Though the model lasts more than a decade, there is only one chance to apply. In response to stakeholder feedback, CMS extended the application deadline to Thursday, December 14 at 11:59 p.m. EST.
The MCP application is non-binding and should take no more than 15 minutes to complete. Eligibility requirements and additional model details can be found in the Request for Applications.
