Grant Application Guidelines

Overview

The Washington Academy of Family Physicians Foundation (WAFP Foundation) is the philanthropic arm of the Washington Academy of Family Physicians (WAFP). The Washington Academy of Family Physicians is a member-driven organization committed to supporting Washington State family physicians in their efforts to provide optimal health care to the people of Washington through education, advocacy, and research. A priority of the WAFP Foundation is to support innovative and high interest programs that reflect our mission.

Selection Criteria
  • Applicants must be a part of medical health related organizations in the state of Washington – and the project must relate to the specialty of Family Medicine.
  • Grant proposals must be consistent with the Foundation mission and goals.
  • Grant proposals must demonstrate collaboration within the medical and general community.
  • Applicants must agree to provide a written summary report at the conclusion of the funded work, which describes the project and its outcome(s).
Selection Process

Grant proposals will be reviewed by the WAFP Foundation Executive Director, and the WAFP Foundation Grants Committee and Board of Directors. Proposals must be submitted on the WAFP Foundation Grant application form. Proposals submitted in other formats will not be considered and will be returned to the applicant.

Application Instructions
  1. DATE OF SUBMISSION
  2. TITLE OF THE PROJECT--Specify the title of the proposed research project. The title should be brief and descriptive of the intent of the proposed project.
  3. NAME & TITLE OF THE APPLICANT, ORGANIZATION OR INSTITUTION--The organization, institution, or individual conducting or sponsoring the proposed project must be identified.
  4. NAME OF FACULTY SPONSOR (if applicant is a student or resident)--Specify the name of a faculty person working with the applicant who is authorized to commit the organization or institution to conduct the study as proposed and ensure the requirements established by the WAFP Foundation are met.
  5. CONTACT INFORMATION: Include contact person and title (if different from the applicant listed above), office address, telephone and fax numbers, and email.
  6. NAME OF INVESTIGATOR(S)--Provide the name each investigator involved in the project. Identify the principal investigator, the one individual designated by the applicant organization to direct the project. If the proposed project is to be under the direction of co-investigators, identify these individuals as "co-investigators."
  7. BUDGET--Specify the amount of WAFP Foundation grant funds being applied for and the total cost of the project (calculate the percent of the total cost the WAFP Foundation funds will represent.) Include other grant funds applied for, already secured, in-kind support and the expected expenses by category.
  8. PROPOSED LENGTH OF TIME TO COMPLETE THE PROJECT--Indicate the anticipated start date and completion date.
  9. HYPOTHESIS—Briefly state the research question and specifically what is expected to be answered by this project.
  10. BACKGROUND & METHODS—Describe the proposed project. What is to be done? What other related research has been done on the topic? How is it to be done? How will its effectiveness be measured? Who will benefit?
  11. ABSTRACT--Summarize the proposed project, in no more than one page, double-spaced, outlining succinctly the objectives and methodology.
  12. APPENDIX--In the event the applicant wishes to provide supporting materials, such as survey instruments or supporting letters, it is requested that a listing be made on the "Appendix" page of the application with the corresponding page numbers for those appendices. If a survey or data collection instrument to be used in the project has been developed, a copy should be enclosed along with any information relative to pre-testing results or assumptions used. We cannot guarantee that materials received after the application has gone to review will be considered if the review has been completed.
  13. BIOGRAPHICAL SKETCH(ES)--The applicant is asked to provide a brief biographical sketch for the principal investigator(s) or, in lieu of a biographical sketch, a copy of his/her curriculum vita (CV) may be attached.
  14. APPLICANT AGREEMENT--Should the grant be awarded, it will be contingent upon agreeing to the following conditions:
    • Grant funds must be used exclusively for the purposes and in the manner set forth in the application. If the funds are not used in keeping with the grant application as approved by the WAFP Foundation, the grant will be voided and the total amount of grant funds awarded returned to the WAFP Foundation upon demand;
    • Unexpended grant funds at the conclusion or suspension of the project will be returned to the WAFP Foundation;
    • Approval from the WAFP Foundation must be obtained, in writing, for any re-budgeting of grant funds prior to any re-allocation of grant funds;
    • Adherence to the timeline set forth in the grant application. If circumstances prevent adherence to the approved timeline, grantee must submit written explanation of those circumstances to WAFP Foundation and set forth exact date of expected project completion.
    • The grantee agrees to appropriately acknowledge the support of the WAFP Foundation in any published reports of the project;
    • The grantee agrees to abide by any additional condition(s) specified at the time an award notification is made by the WAFP Foundation.
